Applicant Checklist

     Two most recent paystubs

     Two most recent year of federal tax returns with ALL schedules attached

     Two most recent W-2’s

     If you own 25% or more in a corporation or partnership, please supply two most recent year’s K-1’s federal business tax returns

     Copy of driver’s license

     Most recent two months statements for any savings, checking, stocks, bonds, IRA’s, 401K’s, etc. **Include every page, even if the page is blank

     If you currently own any properties, please provide:

   Copy of mortgage statement for each property

   Copy of property tax bill for each property

   Copy of homeowner’s insurance declaration page for each property

   Copy of rental/lease agreements, if applicable
